The Home Base Primary Care (HBPC) Medical Director within the Geriatric and Extended Care Service at the Thomas E. Creek VA Health Care System is responsible for clinical over site of all HBPC services in Amarillo and satellite programs.

The HBPC Medical Director is the senior clinician and is responsible for the overall medical care delivered by the HBPC team.

• Works in collaboration with the HBPC Program Director to meet the requirements of the program.
• May have a faculty appointment and be involved in academic activities with affiliated Medical Schools, residency and fellowship programs.
• Provides patient care directly and through the supervision of licensed independent practitioners (LIPs), residents, fellows and medical students in the home setting and in other departments within the GEC Service line.
• Has administrative responsibilities for leadership and direction of HBPC development and programs, as well as effective interface with the HBPC staff.
• Participates fully in all accreditation processes, reviews, and site visits.
• Provide direct patient care which includes assessments, diagnosis, treatment, and outcome measures of healthcare problems. This includes the following: initiating and adjusting therapeutic regimens, performing histories and physicals, ordering diagnostic and therapeutic tests/studies, initiating consults and referrals, and prescribing medications/supplies in accordance with VA policy, VA formulary, Federal & state regulations, and scope of practice. Collaborate with in house specialty clinics as well as community providers.

Work Schedule: Full-time, Monday - Friday, 0800-1630
Driving Requirement: This position has a driving requirement and requires a current, valid state-issued Driver's license

Podiatrist Basic Requirements

• Education: Degree as a doctor of podiatric medicine, or its equivalent, from a school of podiatric medicine approved by the Secretary of Veterans Affairs. Approved schools are United States schools of podiatric medicine approved by the Council on Podiatry Education of the American Podiatry Association in the year in which the degree was granted. ~AND~
• Residency: Completed, or currently completing, residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. VA Podiatrists involved in academic training programs may be required to be board certified for faculty status. Approved residencies are: (1) Those approved by the accrediting bodies for graduate medical education, the Council on Podiatric Medical Education, in the list published for the year the residency was completed, or (2) Other residencies and training experiences, or their equivalents, which the local Professional Standards Board determines to have provided an applicant with appropriate professional training. ~AND~
• License: Currently hold, or will hold, a full and unrestricted license to practice Podiatry in a State, Territory, or Commonwealth of the United States or in the District of Columbia.
• English Language Proficiency: Proficiency in spoken and written English.

Preferred Experience:

• Fellowship trained in Geriatrics
• Board Certificated/Board Eligible in Geriatrics
• Experience in working with Geriatric Veteran population across various clinical settings, to include clinics, nursing homes, home care, consultative practice.
• Experience in Hospice and Palliative care

Physical Requirements: Moderate lifting, 15-44 pounds; Reaching above shoulder; Use of fingers; Both hands required; Walking (5 hours); Standing (5 hours); Kneeling (5 hours); Repeated bending (0.5 hours); Climbing, legs only (2 hours); Operation of vehicle; Hearing (aid may be permitted).

Driving Requirement: Assessment must be made for any acute or chronic medical/physical condition or medication use which interferes with the ability operate the appropriate Government owned or -leased vehicle safely and without undue risk to themselves or others. The following requirements must be met:

• distant vision 20/40 in one eye with or without correction
• field of vision 70 degrees,
• ability to distinguish red, green and amber
• whispered voice at five feet, or average hearing loss of not greater than 40 dbs. at 500, 1000 and 2,000 Hz
