The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in Geriatrics and Extended Care Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. The NP practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position.
The major duties and responsibilities include, but are not limited to:
- Assesses and supports the caregiver's capability to continue to provide care.
- Helps develop and approve patient treatment plans based on a comprehensive interdisciplinary
assessment of the patient and the caregiver.
- Functions as the Primary Care provider for assigned HBPC Veterans and provides clinical oversight of assigned staff.
- Implements the Veteran's treatment plan through home visits.
- Provides education, counseling and emotional support for patients and families receiving care.
- Instructs medical/nursing students, residents, and other learners that rotate through GEC Service.
- Coordinates Veteran care with other members of health care team.
- Obtains and evaluates medical histories, performing physical examinations, therapeutic and diagnostic procedures, and ordering laboratory studies, radiographs, electrocardiograms, and other special examinations.
- All other duties as assigned.
