Duties include, but are not limited to:
- Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
- Influences and models the delivery of evidence- based health care, demonstrating expertise in the ability to manage a patient panel, population, or program.
- Develops and implements an individualized plan to achieve optimal population health and disease management.
- Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
- Orders and interprets diagnostic studies.
- Prescribes pharmacologic and non-pharmacologic interventions.
- Initiates referrals/consults to specialty services
- Provides education to patients and their families.
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
- Leads and implements changes that enhance the quality and timeliness of Veteran/patient care applying research and evidence in the delivery of healthcare for patient populations.
- Initiates and leads interdisciplinary teams informed by structured approaches and processes to improve outcomes that positively impact care for a patient panel, population, or program.
- Acquires knowledge and skills, then disseminates acquired information within and/or outside the medical facility.
- Initiates and leads interdisciplinary teams developing systems that positively impact outcomes at the panel, population, or program level. Demonstrates effective communication skills and professional behavior, promoting cooperation and teamwork.

- Chronic disease management of medical diseases including hypertension, diabetes, ischemic heart disease, heart failure, chronic pain, and chronic obstructive pulmonary disease using clinical practice guidelines.
- Acting as "surrogates" for the patients of colleagues, when one or more of their colleagues are on leave.
- Timely documentation and completing administrative work in a timely fashion.
- Addressing clinical reminders as medically appropriate. Quality of care meeting professional standards.
- Participation in activities/committees/boards/projects as dictated by strategic or other initiatives/taskings.
- Supervising or collaborating physician for a mid-level provider. Provide clinical oversight and review. Supervising or collaborating responsibilities and performance noted in the physician's performance evaluation.
- Assigned a panel of patients to manage.
- Follow departmental and facility policies for leave and attendance and work within appointment guidelines.
- Provide preventive health for both male and female patients.
Preferred Experience:
- 1 year Primary Care experience Preferred
- Skill in interpreting EKG's
- Knowledge of or willingness to learn and incorporate virtual care visits
