NOTE: This position is based out of the EUREKA VA Outpatient Clinic, duties entail traveling to the homes of Veterans living in the surrounding area.
The Home Based Primary Care Nurse Practitioner (HBPCNP) serves veterans with complex, chronic, and disabling diseases who have difficulty traveling to VA facilities for care. HBPC delivers comprehensive, interdisciplinary primary care right in the veteran's home.
Given the rural and geographically isolated setting of Eureka, the HBPCNP plays a critical role in ensuring access to timely, continuous primary care services. The position requires strong clinical judgment, adaptability, and a commitment to serving Veterans in an underserved region while upholding VA standards of care, performance metrics, and regulatory requirements.
The incumbent independently manages a panel of patients with acute and chronic health conditions, with an emphasis on health promotion and disease prevention, using evidence-based practice in the home of Veterans. A valid Driver's License and insurance are required.
Major duties and responsibilities of the position:
- Develops and implements an individualized plan to achieve optimal population health and disease management.
- Demonstrates leadership and works collaboratively with the interdisciplinary team.
- Orders and interprets labs, tests, and diagnostic studies.
- Prescribes pharmacologic and non-pharmacologic interventions.
- Initiates referrals and consults to specialty services.
- Provides education to patients and their families.
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.

- Valid Driver's License and Auto insurance required.
